Semaglutide
Semaglutide is a GLP-1 receptor agonist made by Novo Nordisk, sold as Ozempic and Wegovy weekly injections and as Rybelsus, Ozempic and Wegovy daily tablets.
US regulatory status
FDA-approved prescription drug
Novo Nordisk makes every FDA-approved semaglutide product. Ozempic injection is approved for adults with type 2 diabetes to improve blood sugar, to lower the risk of major cardiovascular events in those with established cardiovascular disease, and to lower the risk of kidney function decline, kidney failure and cardiovascular death in those with chronic kidney disease. Rybelsus and Ozempic tablets share one label: once-daily tablets for adults with type 2 diabetes, to improve blood sugar and to lower the risk of major cardiovascular events in those at high risk, and they are not interchangeable mg for mg. Wegovy injection is approved for long-term weight reduction in adults and children 12 and older with obesity, and in adults with overweight and a weight-related condition; to lower the risk of cardiovascular death, heart attack and stroke in adults with cardiovascular disease and obesity or overweight (since March 2024); and, under accelerated approval since August 2025, for noncirrhotic MASH with moderate to advanced liver fibrosis in adults. Its 7.2 mg dose, Wegovy HD, was approved on March 19, 2026, for weight reduction in adults only. Wegovy tablets, approved on December 22, 2025, are for adults, for weight reduction and heart risk reduction, and are not approved for MASH or for children. Compounded semaglutide is not FDA-approved: the FDA declared the shortage resolved on February 21, 2025, its grace periods for compounders ended in 2025, and on April 30, 2026 it proposed excluding semaglutide from the list outsourcing facilities may compound from (no final decision as of this date). Salt forms such as semaglutide sodium and acetate are different active ingredients from the approved drugs, and the FDA has sent warning letters to sellers of semaglutide labeled “for research purposes” or “not for human consumption”.

What the research shows
Evidence grade
Strong human evidence
Consistent results from large randomized controlled trials in people.
Weight: in STEP 1 (adults with obesity or overweight and no diabetes, 68 weeks), average body weight change was −14.9% on semaglutide 2.4 mg and −2.4% on placebo. In the label’s 72-week trials of the 7.2 mg dose in adults with obesity and no diabetes, it was −18.8% at 7.2 mg, −15.5% at 2.4 mg and −3.9% on placebo. In OASIS 4 (307 adults, 64 weeks), the 25 mg tablet that became Wegovy tablets gave −13.6%, compared with −2.2% on placebo. In STEP TEENS (201 adolescents with obesity, 68 weeks), BMI fell 16.1% on semaglutide and rose 0.6% on placebo. Heart and kidneys: in SELECT (17,604 adults with cardiovascular disease and overweight or obesity but no diabetes), cardiovascular death, heart attack or stroke occurred in 6.5% on semaglutide and 8.0% on placebo (hazard ratio 0.80); the label notes that the effect on cardiovascular death alone was not confirmed and the effect on heart failure is not established. In SUSTAIN-6 (3,297 adults with type 2 diabetes), the same events occurred in 6.6% on Ozempic and 8.9% on placebo (hazard ratio 0.74). In FLOW (3,533 adults with type 2 diabetes and chronic kidney disease, stopped early at a planned interim analysis, median 3.4 years), the risk of a major kidney or cardiovascular event was 24% lower (hazard ratio 0.76); the label adds that the benefit was not evident in people already taking an SGLT2 inhibitor, though there were few events in that group. With tablets, PIONEER 6 (3,183 adults with type 2 diabetes) showed oral semaglutide was not worse than placebo for heart risk (3.8% vs 4.8%), and SOUL (9,650 high-risk adults, 14 mg) found major cardiovascular events in 12.0% vs 13.8% on placebo (hazard ratio 0.86). Liver: in ESSENCE (800 adults with MASH and moderate to advanced fibrosis), at 72 weeks steatohepatitis resolved without worse fibrosis in 62.9% on semaglutide vs 34.3% on placebo, and fibrosis improved without worse steatohepatitis in 36.8% vs 22.4%; the trial continues to 240 weeks to show whether this prevents liver-related events. Novo Nordisk funded all of these trials.
How we grade evidence
Strong human evidence · this page
Consistent results from large randomized controlled trials in people.
Limited human evidence
Some studies in people, but they are small, short, early-phase or uncontrolled.
Mixed evidence
Studies in people disagree, or strong evidence for one use sits beside weak evidence for another.
Animal or lab studies only
Studied in cells or animals, not properly in people. Animal results may not apply to humans.
The grade describes how much research exists and in whom. It is not a judgment that a compound works or is safe.
Side effects and safety
The most common side effects are digestive: nausea, diarrhea, vomiting, constipation and stomach pain, mostly while the dose is being increased. In the Wegovy 2.4 mg weight trials, nausea affected 44% of people (16% on placebo), vomiting 24% (6%) and hair loss 3% (1%), and 6.8% stopped treatment because of side effects (3.2% on placebo). On Ozempic, nausea affected 15.8% at 0.5 mg and 20.3% at 1 mg (6.1% on placebo); on 14 mg tablets, 20% (6% on placebo). Unusual skin sensations such as sensitivity, pain or burning (dysesthesia) rise with the dose: 22% at Wegovy 7.2 mg, 6% at 2.4 mg and 0% on placebo, and 5% on Wegovy tablets; the FDA is investigating this reaction further. Every semaglutide label carries a boxed warning: semaglutide caused thyroid C-cell tumors in rodents, it is unknown whether it does in people, and it must not be used by anyone with a personal or family history of medullary thyroid carcinoma or with MEN 2. Other label warnings include pancreatitis (including fatal cases reported after approval), gallbladder disease, severe stomach and bowel side effects, kidney injury from dehydration, low blood sugar with insulin or drugs that raise insulin release, a faster resting heart rate, serious allergic reactions and aspiration during anesthesia or deep sedation. In people with type 2 diabetes, diabetic eye complications were more frequent on semaglutide in SUSTAIN-6 (hazard ratio 1.76). Pens must never be shared, even with a new needle. The FDA had received 990 adverse event reports for compounded semaglutide as of May 31, 2026, and has reported overdoses from dosing errors, some needing hospital care, when compounded doses were measured in syringe units.

Questions people ask
Is semaglutide the same as Ozempic?
Semaglutide is the active ingredient in Ozempic, and also in Wegovy and Rybelsus. The products differ in form and approved use: Ozempic injection, Ozempic tablets and Rybelsus are for type 2 diabetes, and Wegovy injection and tablets are for weight reduction and heart risk. They are not swapped freely: Rybelsus and Ozempic tablets are not interchangeable mg for mg, and the Wegovy label says not to combine it with any other semaglutide product or GLP-1 medicine. Compounded semaglutide is not the same as any of them: it is not FDA-approved, and some compounders have used salt forms (semaglutide sodium or acetate) that are different active ingredients.

What are the side effects of Ozempic?
On the Ozempic injection label, the most common are nausea (15.8% at 0.5 mg and 20.3% at 1 mg, vs 6.1% on placebo), vomiting, diarrhea, stomach pain and constipation. Ozempic tablets and Rybelsus add decreased appetite, and their Medication Guide says nausea, vomiting and diarrhea are most common when you first start. The Wegovy label lists the same digestive effects plus headache, tiredness, dizziness, burping, skin sensitivity and hair loss. Stop and call your prescriber for severe stomach pain that won’t go away, sometimes spreading to your back: it can be pancreatitis.

Does semaglutide cause hair loss?
It can, and the Wegovy label links it to weight reduction. In the Wegovy 2.4 mg weight trials, hair loss was reported by 3.3% of people (4% of women, 0.9% of men), compared with 1% on placebo. At the 7.2 mg dose it was 5.8% (8.4% of women, 0.2% of men). Tell your prescriber if it bothers you.
How long can you take semaglutide?
Wegovy is approved to reduce body weight and “maintain weight reduction long term”, and your prescriber decides how long you stay on any semaglutide product. Safety data go as far as the trials: a median of about 3.5 years in SELECT, 3.4 years in FLOW and about 4 years in SOUL. In SELECT, 16.6% stopped semaglutide because of side effects, compared with 8.2% on placebo. Effects beyond that are not known. The label warnings that matter over time are gallbladder problems, pancreatitis, diabetic eye complications and thyroid C-cell tumors, which semaglutide caused in rodents (the risk in people is unknown).

Is compounded semaglutide FDA-approved?
No. Compounded drugs are not FDA-approved, so the FDA does not check their safety, effectiveness or quality. The semaglutide shortage ended on February 21, 2025, and the FDA has since limited when pharmacies may compound it. It has warned that salt forms have no known lawful basis, and it has reported overdoses of 5 to 20 times the intended dose when people drew compounded semaglutide from vials in “units”, for example 50 units instead of 5 on a 1 mL (U-100) syringe. It advises discarding a multi-dose vial 28 days after first use. If you use a vial, the semaglutide units calculator converts the dose your prescriber gave you into syringe units at your vial’s concentration.

What is the half-life of semaglutide?
About 1 week, on every semaglutide label. After the last dose, semaglutide stays in the blood for about 5 weeks (Ozempic injection and the diabetes tablets) to 5 to 7 weeks (Wegovy 2.4 mg or 7.2 mg injection, or 25 mg tablets). That is why the labels advise stopping at least 2 months before a planned pregnancy, and why side effects of an overdose may need a long period of observation.
