GLP-1 and glucagon receptor dual agonist

Mazdutide

Mazdutide is a once-weekly injection that acts on GLP-1 and glucagon receptors, approved in China for weight management and type 2 diabetes but not by the FDA.

US regulatory status

Investigational, not approved

Not approved by the FDA for any use. China’s drug regulator (NMPA) approved mazdutide for chronic weight management (announced June 2025) and for type 2 diabetes (announced September 2025); it is sold there by Innovent Biologics under license from Eli Lilly. Lilly has published a US phase 2 trial, and we found no US phase 3 trial or FDA filing as of October 16, 2026. The FDA has warned companies selling mazdutide falsely labeled “for research purposes.”

What the research shows

Evidence grade

Limited human evidence

Some studies in people, but they are small, short, early-phase or uncontrolled.

Most phase 3 evidence comes from China. In GLORY-1, 610 Chinese adults with obesity or overweight (average weight 87.2 kg) changed weight by −11.00% and −14.01% over 48 weeks on the two doses tested, while placebo averaged a 0.30% gain; 35.7% and 49.5% lost at least 15%, compared with 2.0%. In GLORY-2, 461 Chinese adults with a BMI of 30 or more changed weight by −16.65% over 60 weeks, compared with −1.50% on placebo. In Lilly’s US phase 2 trial in 179 adults without diabetes, weight change at 32 weeks ranged from −7.3% to −18.1% across the three doses tested, compared with −0.9%. In type 2 diabetes, mazdutide lowered HbA1c by 1.57 and 2.15 points over 24 weeks, compared with 0.14 on placebo (DREAMS-1), and lowered HbA1c and weight more than dulaglutide over 28 weeks (DREAMS-2). Innovent funded GLORY-1 and Lilly funded the US trial.

How we grade evidence
  1. Strong human evidence

Consistent results from large randomized controlled trials in people.

  2. Limited human evidence · this page

Some studies in people, but they are small, short, early-phase or uncontrolled.

  3. Mixed evidence

Studies in people disagree, or strong evidence for one use sits beside weak evidence for another.

  4. Animal or lab studies only

Studied in cells or animals, not properly in people. Animal results may not apply to humans.

The grade describes how much research exists and in whom. It is not a judgment that a compound works or is safe.

Side effects and safety

Stomach and bowel side effects were the most common in every trial, mostly mild to moderate. In GLORY-2, vomiting (53.1% vs 1.3% on placebo), nausea (46.9% vs 3.2%) and diarrhea (39.4% vs 6.5%) were frequent, and 2.9% stopped because of side effects, compared with none on placebo. In the US phase 2 trial, stopping treatment because of side effects was most frequent at the highest dose tested, at 20%. Against dulaglutide, stomach and bowel side effects were more common with mazdutide. Mazdutide has no FDA label, so there is no US list of warnings or of who should not use it. The FDA says products sold as “research” mazdutide are of unknown quality and may be harmful.

Questions people ask

Is mazdutide approved?

In China, yes; in the US, no. Innovent Biologics announced on June 27, 2025 that China’s NMPA approved mazdutide for chronic weight management in adults with overweight or obesity, based mainly on a phase 3 trial in Chinese adults, and in September 2025 that it was approved for blood sugar control in type 2 diabetes. No mazdutide product appears on the FDA’s 2026 list of novel drug approvals, and we found no FDA filing as of October 16, 2026.

Who makes mazdutide, and how does it work?

Mazdutide acts on two hormone receptors, GLP-1 and glucagon, and is injected under the skin once a week. Eli Lilly develops it as LY3305677, and Innovent Biologics, which holds an exclusive license from Lilly for China, develops it there as IBI362. The published phase 3 trials are Innovent’s, in Chinese adults; Lilly has published a US phase 2 trial, and we found no US phase 3 trial.

How much weight did people lose on mazdutide in trials?

In GLORY-1, Chinese adults lost 11.00% and 14.01% of their weight on average over 48 weeks on the two doses tested, while placebo averaged a 0.30% gain. In GLORY-2, adults with a BMI of 30 or more lost 16.65% over 60 weeks, compared with 1.50% on placebo, and 84.3% lost at least 5%, compared with 33.1%. In Lilly’s smaller US phase 2 trial, weight change at 32 weeks was −7.3%, −15.6% and −18.1% across the three doses tested, compared with −0.9%. Trials differ in who took part and for how long, so figures from one trial don’t transfer directly to another.

What are the side effects of mazdutide?

Mostly digestive. In GLORY-2, vomiting affected 53.1% of people on mazdutide and 1.3% on placebo, nausea 46.9% and 3.2%, and diarrhea 39.4% and 6.5%. In DREAMS-1, the most common side effects were diarrhea, decreased appetite and nausea. Stopping because of side effects was uncommon in GLORY-1 and GLORY-2 (0.5% to 2.9% on mazdutide), but reached 20% at the highest dose in the US phase 2 trial. Without an FDA label, there is no US review of its full safety profile.

Is mazdutide used for type 2 diabetes?

In China, yes: it was approved for blood sugar control in adults with type 2 diabetes in September 2025. In DREAMS-1, 320 Chinese adults managing diabetes with diet and exercise lowered HbA1c by 1.57 and 2.15 points over 24 weeks, compared with 0.14 on placebo, and weight by 5.61% and 7.81%, compared with 1.26%. In DREAMS-2, 731 adults were compared with dulaglutide (Trulicity) for 28 weeks: both doses tested lowered HbA1c (by 0.24 and 0.30 points more) and weight (by 3.78% and 5.76% more) than dulaglutide, with more stomach and bowel side effects.

Is mazdutide being studied for alcohol use disorder?

Yes, in one trial so far. Eli Lilly ran a phase 2 trial of mazdutide against placebo in adults with alcohol use disorder (NCT06817356); it is listed as completed, with no findings posted as of October 16, 2026. Mazdutide is not approved for this use anywhere.
