How many units is 15 mg of tirzepatide?

The highest labeled tirzepatide dose in U-100 units at example strengths, what a full small syringe holds, and how Zepbound and Mounjaro measure it in mL.

How many units 15 mg of tirzepatide is depends entirely on your vial’s strength: on a U-100 syringe, units = mg ÷ (mg/mL) × 100, so the answer changes with every mg/mL. Find the strength on your vial or pharmacy label and read its row below, or enter it in the tirzepatide units calculator.

15 mg is also the largest amount on either FDA label, which makes two checks worth doing every time: that your syringe holds the whole volume, and that the amount you draw is the one your prescriber wrote.

15 mg in units at example strengths

These strengths are examples, not recommendations. Compounded vials come in whatever strength the pharmacy made, so units never carry over from one vial to another.

15 mg at different strengthsArithmetic
How many mL and U-100 units 15 mg is at each strength.
StrengthVolumeUnits (U-100)
15 mg/mL1 mL100 units
20 mg/mL0.75 mL75 units
25 mg/mL0.6 mL60 units
30 mg/mL0.5 mL50 units

The same amount is a different number of units at each strength, so check the mg/mL on your vial or pharmacy label. U-100 syringe: 100 units = 1 mL.

Every row is more than a 30-unit syringe holds. Below the highest strength it is more than a 50-unit syringe holds too, and at the lowest strength in the table it fills a whole 1 mL syringe.

What a full syringe holds

The other way to see it: here is how much tirzepatide sits in a full 30, 50 and 100-unit syringe at one example strength, 25 mg/mL.

What each reading holds at 25 mg/mLArithmetic
The amount in each syringe reading at 25 mg/mL.
ReadingVolumeAmount
30 units0.3 mL7.5 mg
50 units0.5 mL12.5 mg
100 units1 mL25 mg

U-100 syringe: 100 units = 1 mL. At a different strength the same reading holds a different amount.

At this example strength, a full 30-unit syringe holds half of 15 mg, and a full 50-unit syringe still falls short of it. If your syringe’s highest mark is lower than the units for your strength, stopping there gives you less than your dose. Ask your pharmacist which syringe fits your dose in one draw.

Where 15 mg sits on the labels

Both labels climb in 2.5 mg steps (2.5, 5, 7.5, 10, 12.5, 15 mg once weekly), each step after at least 4 weeks on the current dose. 15 mg is the sixth and last step.

  • Zepbound: “The maximum dosage of ZEPBOUND for all indications is 15 mg injected subcutaneously once weekly.” It is also one of the recommended maintenance dosages, both for weight reduction (“5 mg, 10 mg, or 15 mg”) and for obstructive sleep apnea (“10 mg or 15 mg”) (Zepbound label, section 2.2).
  • Mounjaro: “The maximum dosage of MOUNJARO is: 15 mg injected subcutaneously once weekly in adults. 10 mg injected subcutaneously once weekly in pediatric patients.” So 15 mg is for adults only (Mounjaro label, section 2.1).

These are label facts, not advice. Follow your prescriber: they set your dose and when it changes.

How the labeled products give 15 mg

Zepbound and Mounjaro list the same 15 mg presentations. All are ready-made liquids measured in mg and mL, never in units, and none needs water:

PresentationWhat the label saysHow you measure it
Single-dose pen15 mg/0.5 mLThe pen gives the whole dose
Single-dose vial15 mg/0.5 mLDraw to the 0.5 mL line
Multi-dose vial60 mg/2.4 mL (25 mg/mL), four 15 mg doses of 0.6 mLDraw to the 0.6 mL line
KwikPen60 mg/2.4 mL, four fixed dosesTurn the dose knob until it stops

The multi-dose vial and KwikPen hold the highest total of any tirzepatide presentation: four maximum doses. Once opened, the label says to throw either away “after a total of 30 days at room temperature, 30 days after first use, or after taking 4 weekly doses, even if there is medicine left in it” (Zepbound label, sections 3 and 16.2). For vials, both labels name “a 1 mL syringe capable of measuring a 0.5 mL or 0.6 mL dose” (Mounjaro label, section 2.2).

Questions people ask

How many units of Zepbound is 15 mg?

None: Zepbound isn’t measured in units. The 15 mg pen gives the whole dose, the single-dose vial holds 15 mg in 0.5 mL, and the multi-dose vial gives each 15 mg dose as 0.6 mL on a 1 mL syringe (Zepbound label). Units only apply when your instructions are written for an insulin syringe; the 2.5 mg answer explains why.

Is 15 mg the highest dose of tirzepatide?

On the FDA labels, yes. Zepbound’s maximum for all indications is 15 mg once weekly, and Mounjaro’s is 15 mg once weekly for adults (10 mg for children). Neither label goes higher. Your prescriber decides whether you reach it.

Can a 50-unit syringe hold 15 mg?

Only at a high strength. In the table above, most rows need more than 50 units. A 50-unit syringe holds 0.5 mL, so check the units for your strength against it before you draw, or use the tirzepatide units calculator with your syringe size.

How many mg is 15 units of tirzepatide?

It depends on the strength of your vial, and it’s far less than 15 mg. On a U-100 syringe, 100 units is 1 mL, so 15 units is 0.15 mL (FDA). Multiply 0.15 mL by your strength: 2.25 mg at 15 mg/mL, 3 mg at 20 mg/mL, 3.75 mg at 25 mg/mL and 4.5 mg at 30 mg/mL. Labeled Zepbound and Mounjaro aren’t measured in units: the 60 mg/2.4 mL multi-dose vial and KwikPen give four 15 mg doses of 0.6 mL each (Zepbound label).
