Quick answers

How many units is 0.5 mg of semaglutide?

Your vial’s strength decides it. See semaglutide in U-100 units at example compounded strengths, how Ozempic and Wegovy give this dose, and an FDA warning.

How many units 0.5 mg of semaglutide is depends entirely on your vial’s strength: on a U-100 syringe, divide 0.5 mg by the mg/mL and multiply by 100. Look up your vial’s mg/mL in the table below, or type it into the semaglutide units calculator along with your syringe size.

If you use Ozempic or Wegovy, you won’t see units anywhere. Ozempic’s pen counter shows mg, and Wegovy’s 0.5 mg pen and syringe each hold one fixed dose.

0.5 mg in units at example strengths

The strengths in this table are what a 5 mg or 10 mg vial makes with 1, 2 or 2.5 mL of water, as on the semaglutide 5 mg and semaglutide 10 mg pages. Treat them as examples only: your pharmacy’s label is the only source for your vial’s strength.

0.5 mg at different strengthsArithmetic
How many mL and U-100 units 0.5 mg is at each strength.
StrengthVolumeUnits (U-100)
2 mg/mL0.25 mL25 units
2.5 mg/mL0.2 mL20 units
4 mg/mL0.125 mL12.5 units
5 mg/mL0.1 mL10 units
10 mg/mL0.05 mL5 units

The same amount is a different number of units at each strength, so check the mg/mL on your vial or pharmacy label. U-100 syringe: 100 units = 1 mL.

If your vial’s strength stays the same, 0.5 mg takes exactly twice the volume of 0.25 mg; compare with 0.25 mg of semaglutide in units. If a refill comes at a different mg/mL, every reading changes, so check the label on each new vial.

Where 0.5 mg sits on the labels

0.5 mg plays a different part on each label:

  • Ozempic: “After 4 weeks on the 0.25 mg dosage, increase the dosage to 0.5 mg once weekly.” It can also be a maintenance dose: “The recommended maintenance dosage is 0.5 mg, 1 mg, or 2 mg, injected subcutaneously once weekly, based on glycemic control” (Ozempic label).
  • Wegovy: 0.5 mg covers weeks 5 through 8 of Table 1, a “Dosage Escalation” step before 1 mg. The label adds: “If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks” (Wegovy label).

The tablet label also names this injection dose: “Patients taking the 0.5 mg dose of OZEMPIC injection may switch to RYBELSUS tablets,” starting the tablets one week after the last injection (Rybelsus and Ozempic tablets label). Tablets are dosed in mg too, never in units.

These are label facts, not a plan for you. Follow your prescriber on your dose and on any switch.

How the branded products give 0.5 mg

  • Ozempic pen (2 mg/3 mL, 0.68 mg/mL): holds “4 doses of 0.25 mg and 2 doses of 0.5 mg or 4 doses of 0.5 mg.” You “Turn the dose selector until the dose counter stops and shows your dose (0.25 mg or 0.5 mg).”
  • Ozempic single-dose prefilled syringe: 0.5 mg per 0.5 mL.
  • Wegovy single-dose pen or syringe: 0.5 mg/0.5 mL. The pen is “for 1 time use only.”

Ozempic’s pen is single-patient-use, and its label is blunt: “OZEMPIC pens must never be shared between patients, even if the needle is changed” (Ozempic label; Wegovy label).

Questions people ask

How many units of Ozempic is 0.5 mg?

Ozempic has no units. You dial 0.5 mg on the 2 mg pen (“Only doses of 0.25 mg or 0.5 mg can be selected with the dose selector”), or use a 0.5 mg single-dose prefilled syringe. Read the dose counter, and don’t count clicks (Ozempic label).

How many units of Wegovy is 0.5 mg?

Wegovy isn’t measured in units either. Its 0.5 mg pen and syringe each hold 0.5 mg in 0.5 mL, and the pen’s dose “is already set.” The multi-dose FlexTouch pen only gives 2.4 mg doses, so it isn’t used for 0.5 mg (Wegovy label).

How many units of compounded semaglutide is 0.5 mg?

Use the row for your vial’s mg/mL above, or the calculator. Compounded semaglutide isn’t FDA-approved; see its regulatory status. For multi-dose vials the FDA says: “Discard the vial within 28 days after the first use, even if medication remains in the vial,” and “Always use a new, sterile needle and syringe for each dose” (FDA).

What syringe should I use for 0.5 mg?

The one your pharmacist or prescriber gives you. The FDA encourages them “to provide patients with the appropriate syringe size for the intended dose and counsel patients on how to measure the intended dose using the syringe” (FDA). The syringe reader shows what each line is worth on yours.
