How many units is 5 mg of tirzepatide?

Tirzepatide in U-100 syringe units at example strengths, where this amount sits on the Zepbound and Mounjaro labels, and why your syringe size matters.

On a U-100 insulin syringe, 5 mg of tirzepatide is 5 ÷ your vial’s mg/mL × 100 units, so there is no single answer: a vial twice as strong needs half the units. Check the mg/mL on your vial or pharmacy label, then find its row below or enter it in the tirzepatide units calculator.

A Zepbound or Mounjaro pen gives a fixed dose, so there is nothing to convert. Units only matter when you draw from a vial with an insulin syringe.

5 mg in units at example strengths

These concentrations are examples, not recommendations. The 10 mg/mL row is the same concentration as the labeled 5 mg/0.5 mL single-dose vial.

5 mg/mL1 mL100 units
10 mg/mL0.5 mL50 units
15 mg/mL0.333 mL33.3 units
20 mg/mL0.25 mL25 units

The same amount is a different number of units at each strength, so check the mg/mL on your vial or pharmacy label. U-100 syringe: 100 units = 1 mL.

If you just moved up from 2.5 mg on the same vial, your mark moves too: on one vial, 5 mg is exactly twice the units of 2.5 mg. The 2.5 mg answer has the earlier step, and the next one is 7.5 mg in units.

Will 5 mg fit in your syringe?

This is the step where syringe size starts to matter. A 30-unit syringe holds 0.3 mL, a 50-unit syringe 0.5 mL and a 100-unit syringe 1 mL. Here is what each one holds when full, at 10 mg/mL:

ReadingVolumeAmount
30 units0.3 mL3 mg
50 units0.5 mL5 mg
100 units1 mL10 mg

U-100 syringe: 100 units = 1 mL. At a different strength the same reading holds a different amount.

At that strength, 5 mg is more than a full 30-unit syringe. The same goes for any row in the first table above 30 units, and rows above 50 units won’t fit a 50-unit syringe either. If the volume is more than your syringe holds, don’t improvise a second draw: ask your pharmacist for a syringe that fits the dose. The tirzepatide units calculator tells you when a dose is over the syringe you pick.

Where 5 mg sits on the labels

5 mg is the second step of both FDA-label schedules for tirzepatide, and the labels treat it differently from there:

  • Zepbound: “After 4 weeks, increase the dosage to 5 mg injected subcutaneously once weekly.” For weight reduction it is also one of the label’s maintenance options: “The recommended maintenance dosage is 5 mg, 10 mg, or 15 mg.” For sleep apnea, the label lists only 10 mg or 15 mg (Zepbound label).
  • Mounjaro: the dose goes from 2.5 mg to 5 mg only “if additional glycemic control is needed,” after “at least 4 weeks on the current dose.” The Mounjaro label names no fixed maintenance doses (Mounjaro label).

These are label facts, not a plan for you. Follow your prescriber, who decides whether and when your dose changes.

How the labeled products hold 5 mg

Zepbound and Mounjaro have the same four presentations for a 5 mg dose, all labeled in mg and mL:

PresentationLabel strengthHow you measure it
Single-dose pen5 mg/0.5 mLThe pen gives the whole dose
Single-dose vial5 mg/0.5 mL (10 mg/mL)Draw to the 0.5 mL line
Multi-dose vial20 mg/2.4 mL (8.33 mg/mL), four 5 mg doses of 0.6 mLDraw to the 0.6 mL line
KwikPen20 mg/2.4 mL, four fixed 5 mg dosesTurn the dose knob until it stops

The “20 mg” on a multi-dose vial or KwikPen is what the whole cartridge holds, not your weekly dose. It is already a liquid: every FDA-approved tirzepatide product is a ready-made solution, and no label mentions adding bacteriostatic water. The printed 8.33 mg/mL is rounded, so draw labeled vials to the 0.6 mL line rather than to a unit count worked out from it. For vials, both labels name “a 1 mL syringe capable of measuring a 0.5 mL or 0.6 mL dose” (Zepbound label; Mounjaro label).

Questions people ask

How many units of tirzepatide is 5 mg?

Divide 5 by your vial’s mg/mL and multiply by 100. The table above shows the result at four example strengths, and the calculator works it out for yours. If your strength isn’t printed on the vial or your instructions, ask the pharmacy before you draw.

How many units of Zepbound is 5 mg?

Zepbound is labeled in mL, not units. The single-dose pen gives the full 5 mg, the single-dose vial holds 5 mg in 0.5 mL, and the multi-dose vial gives each 5 mg dose as 0.6 mL on a 1 mL syringe (Zepbound label). Follow the Instructions for Use for the presentation you were dispensed.

How many units of compounded tirzepatide is 5 mg?

It depends on the strength the pharmacy made, so units from a friend’s vial or an online chart don’t carry over to yours. Compounded drugs are not FDA-approved, and the FDA does not verify their safety, effectiveness or quality before they are marketed (FDA). See tirzepatide’s regulatory status.

Why did my units double when I went from 2.5 mg to 5 mg?

Because units measure volume. On the same vial, twice the mg is twice the volume, so twice the units. Your syringe has to hold the new volume too, which is where a 30-unit syringe can fall short.
